Remote Patient Monitoring

Your care team, closer to you β€” every single day. A smart device sends your readings to our pharmacists in real time, so small issues get caught before they become big ones.

  • Real-time monitoring
  • Covered by most insurance
  • No Wi-Fi required

The best way to manage your health is to stay ahead of it

Remote patient monitoring brings your care team closer to you, every single day. No paper logs. No remembering numbers. The moment you take a reading, a clinical professional sees it β€” and calls you if anything looks concerning.

It's a simple way to turn an everyday device into real, ongoing care β€” without an extra trip to the clinic.

What we monitor

What we can keep an eye on

Your care team picks the device that fits what you're managing. Most people start with a single one; if you're keeping tabs on more than one condition, we can set you up with a few. Every device is FDA-cleared and clinically validated β€” from trusted brands like iHealth, Welch Allyn, Smart Meter, and Tenovi.

  • Blood pressure

Curious whether monitoring is a fit for your condition? Ask our pharmacy team β€” we'll walk you through it.

How it works

From your home to your care team

Four simple steps β€” and you only do the first one.

1

Your smart device

A connected blood pressure cuff, glucometer, scale, or similar device β€” yours to use at home, just like a regular one.

2

It sends itself

Each reading transfers automatically over Bluetooth or built-in cellular. No app, no Wi-Fi, and nothing to remember.

3

A pharmacist reviews it

Your reading lands in the ClearRxM portal in real time, where one of our pharmacists looks it over.

4

Your care team acts

If a reading needs attention, we contact you directly and alert your primary care provider so action can be taken.

More than just a device

Plenty of plans hand out a "free" device. The difference is what happens to the reading after you take it.

A "free" insurance device

  • The reading stays on the screen
  • No professional reviews it
  • It's a tool β€” not a service

Our monitored program

  • Your reading is sent to your pharmacist
  • A clinical professional reviews it
  • Professional oversight is included

Common questions

What exactly is remote patient monitoring?

It's a clinical service where we provide you with a smart device β€” like a blood pressure cuff or glucose monitor β€” that automatically sends your readings to our pharmacists in real time. No more paper logs or trying to remember your numbers for your next doctor's visit.

Which conditions does it help manage?

It is especially valuable for chronic conditions such as hypertension, diabetes, heart failure, COPD, and obesity. It is also commonly used after a hospital discharge to help ensure a smooth, safe recovery at home.

What happens if my reading is out of range?

Our system flags any reading outside your personalized thresholds immediately. A pharmacist reviews the alert, contacts you directly, and notifies your primary care physician so appropriate action can be taken right away β€” often before a serious problem develops.

Is my doctor involved?

Absolutely. Think of us as your doctor's daily eyes and ears. Every month we send a comprehensive clinical report to your doctor so they have the most accurate data to manage your care. If we see a concerning reading, we contact you and your doctor immediately.

Why is my pharmacy doing this instead of my doctor?

You likely see your pharmacist far more often than your doctor. Because we manage your medications, we are in a great position to see how they are working in your body day to day β€” so this partnership helps make sure your treatment is working exactly as intended.

Is the technology hard to use?

Not at all. You use the device exactly as you would any standard blood pressure cuff, scale, or glucometer. The data is sent automatically β€” via Bluetooth or built-in cellular β€” to a secure dashboard. No smartphone, Wi-Fi, or technical know-how is required.

Is it covered by insurance? What will it cost me?

For most patients with Medicare or commercial insurance, remote patient monitoring is a covered benefit β€” and many pay little to nothing out of pocket. We bill your insurance directly, and before you enroll we check your specific plan and tell you exactly what, if anything, it will cost β€” so you can decide with the full picture.

How do I get started?

Just ask any member of our pharmacy team. We will verify your insurance coverage, go over any costs with you up front, and schedule a short 10-minute in-store demonstration so you feel completely comfortable before taking the device home.

Our remote monitoring runs on ClearRxM β€” a pharmacist-built, CPESN-approved remote patient management platform from ClearTec Health, a business our family helped build. It's what lets your pharmacist see your readings in real time and keep your doctor in the loop. The monitoring program is provided through My Dr's Pharmacy, right here at 492 Elden Street.

Your care team reviews your data and will contact you and your doctor if any reading requires attention. Insurance coverage is verified individually before enrollment.
