Moderna COVID-19 vaccine newest formulation (2024) is available now at My Dr's Pharmacy for anyone 6 months of age or older.
Vaccines available currently include:

Pediatric COVID vaccines for anyone under three years of age will be administered by nurses.
Most of our other vaccines are administered by nurses.

When scheduling your appointment, please make sure you have the relevant Medical and Prescription insurance cards or Medicare card with you. Below are images sample cards showing information we need you to provide when scheduling your appointment.

Please keep in mind, that you only need to provide your Medicare information if you are Medicare eligible.

If you've previously made an appointment with us or received medication services, we only require your full name, address, email, phone number, and date of birth. Feel free to leave other fields blank for your convenience.
WHAT DO I BRING TO MY APPOINTMENT?

On the day of your appointment please make sure to bring the following:

Valid government issued ID.

Relevant prescription and medical insurance cards (if you have Medicaid or private health insurance).

If you have Medicare, bring the original government issued Medicare card (the red, white, and blue card) in addition to your prescription (Part D) card.
